Responsibilities:
- Provide direct patient care in the home health setting - Conduct comprehensive assessments of patients' health status - Develop and implement individualized care plans - Administer medications and treatments as prescribed by physicians - Monitor and evaluate patients' progress and make necessary adjustments to care plans - Educate patients and their families on managing their health conditions - Coordinate with other healthcare professionals to ensure continuity of care - Document all patient interactions and interventions accurately and timely
Experience:
- Minimum of 2 years of experience as a Registered Nurse, preferably in home health or a related field - Proficient in performing various nursing procedures such as ostomy care, dermal fillers, sterile processing, sonography, spinal tap, gastric lavage, and esthetic laser treatment - Familiarity with eClinicalWorks or similar electronic medical record systems is a plus - Strong assessment and critical thinking skills to provide quality patient care - Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members We offer competitive compensation and benefits packages for our Registered Nurses.
