Assessments.
Responsible for coordinating and maintaining schedules for clients and staff.
Identifies payer information and verifies.
Obtains authorizations.
Collects and maintains statistical data on all referrals and admissions and reports information daily, weekly, monthly, and as requested.
Maintain the care givers records.
Follow up with clients on a regular basis.
Other tasks and duties as assigned by the supervisor.
CUSTOMER SERVICE Answers telephone lines promptly and efficiently.
Responds to message content appropriately; disseminates information to the appropriate persons.
Surveys patients/families to determine level of satisfaction with agency services.
Documents findings and notifies appropriate supervisor of any dis-satisfied customers.
ORGANIZATIONAL RESPONSIBILITIES Prioritizes work effectively and is acceptably productive.
Maintains an acceptable attendance record according to agency policy.
Accepts responsibility for behavior and activity.
Participates in agency's quality improvement program.
Interacts collaboratively with all team members.
Maintains confidentiality of patient and agency information at all times.
