Clinical Case ManagerJOB SUMMARYThe Clinical Case Manager supports 50
sub-contracting providers in providing behavioral health and therapy services through Kaiser Permanente.
This position can support the Walnut Creek or San Jose office.
The Clinical Case manager liaises between the clients, departments and resources to assess and coordinate client treatment and therapy plans, and is responsible to oversee the administration of coordinated care to ensure that each client receives high quality services.
The Case Manager would review all reports prior to submission to ensure that required information is appropriately documented.
ESSENTIAL JOB FUNCTIONSConduct clinical and psycho-social assessments.
Facilitate intakesResponsible for the development of a comprehensive and clinically appropriate care plan in collaboration with the interdisciplinary team.
Follows care plan through effective health coaching and risk managementResponsible for assessing and addressing risk, and escalates high risk clinical concerns to supervisorServes as a liaison between client and other departmentsResponsible for the final review, approval and submission of all clinical reports to the funderWorks with providers to make appropriate revisions needed on clinical reportsServes as the main contact for clients and families and responds to all client inquiries ; responsible to ensure client's needs are metMakes decisions regarding authorization extension requests for clientsResponsible for providing resolution to client concerns, including provider change requests as appropriate.
Completes documentation regarding all interactions with the client, provider and interdisciplinary team, using appropriate systems and procedures in a timely manner, per agency policies.
Assigns clients to appropriate ABA and/or Therapy Providers for servicesResponds to all funder inquires involving their clientCollaborates with the provider regarding clinical concerns related to their clientDocuments all client complaints and grievances, and submits these to the Quality department, per policy requirementsSubmits all client related Reportable Event forms from providers to the the BHPN Quality department.
Attends staff meetings, in-services, trainings, and other meetings as requested by managementParticipate in special projects, and assist with additional duties or tasks, as assigned by supervisorMust adhere to all federal, state and local laws/legislations as applicable as well as HIPAA laws and regulatory agenciesAttaining and achieving position competencies in relation to role responsibilitiesQUALIFICATIONSMinimum Education, Experience & Training Equivalent to:
Master's degree from an accredited college or university in social work, counseling or marriage family therapy is requiredLicensed Clinical Psychologist, LCSW, MFT, ASW or MFTI PreferredOne to two years of related professional experience working with children with autism spectrum disorders (ASD) and/or other related developmental disabilities in a multi-disciplinary team setting preferredKnowledge of community resources and experience networkingPrevious experience of case management or clinical experience, and responsibility for a large caseload for an extended period of time.
Demonstrated expertise in crisis management.
Knowledge, Skills & Abilities:
Familiar with the field of early intervention, ABA, and knowledgeable of other community resources and agencies that serve children.
Able to conduct strengths based, client centered psychosocial assessments and refer clients for clinically appropriate servicesAble to consistently demonstrate good judgment and decision-making skills.
Able to appropriately interpret and implement policies, procedures, and regulations.
Excellent coalition building skills with the ability to communicate and work effectively with a variety of internal and external stakeholders.
Able to work with sensitive information and maintain confidentiality and have a high level of professionalism.

Able to travel to multiple work sites if needed.

Physical Requirements:
Frequent bending, reaching, squatting, kneeling, twisting in order to observe, assess, and interact with clients.
Ability to stand or sit for extended periods of time, stand for up to 6-8 hours a day.
Frequent speaking and listening (50%) to clients, staff, and other professionals in meetings and on the phone.
Frequent driving (to and from office and client homes)Occasional sitting and maintaining close visual attention to write reports and work at the computer.
Occasional lifting, carrying, and loading/unloading toys and materials up to and including 25lbs to 50lbs used in home visits.
Ability to physically implement behavior management strategies including responding to physically aggressive behavior.
Visual and auditory ability to work with clients, staff and others in the workplace continuously.
Ability to utilize computer, cell phone (iphone), fax machine, telephone and copy machine.
Frequent proofreading and checking documents for accuracy.
Must be able to communicate effectively within the workplace, read and write using the primary language within the workplace.
CONDITIONS OF EMPLOYMENTAbility to obtain and maintain criminal record clearance through the Department of Justice (DOJ).
The People & Performance Department must analyze DOJ/FBI live scan reports in accordance with applicable Federal, State, and Local laws, as well as fitness for the position.
Must have a valid California Driver's License with Insurance, and maintain a clean driving record.
Must pass health screenings, obtain vaccinations and clear TB testing based on company policies.
Ability to obtain and maintain clearance through the Office of Inspector General.
Ability to obtain and maintain CPR certificate.
Ability to obtain and maintain a National Provider Identifier (NPI).
Must attend all BHPN required training.
